MGA Home Healthcare Notifies Individuals of Health Information Breach
MGA Home Healthcare Colorado, Inc. is currently notifying potentially affected individuals that a laptop containing patient health information was stolen from an employee’s locked vehicle between Aug. 19, 2016 and Aug. 20, 2016. MGA discovered the theft on Aug. 20, 2016, and has been conducting a thorough review of the potentially affected records to confirm what information was exposed. A report has been filed with law enforcement. MGA also has taken steps to ensure that a breach like this does not occur again in the future.
The stolen laptop included information of approximately 3,119 patients (and some employees). The information on the laptop included names, addresses and other demographic information, and (with respect to patients) information about the health care services that MGA provided. The laptop also included Social Security numbers or driver’s license numbers of approximately 32 patients.
MGA has no evidence that the information on the laptop has been accessed or used. As a precaution, MGA is offering identity theft protection services to affected individuals. Individuals will receive a letter from MGA (if MGA has the patient’s current address), or can call the below phone number to confirm whether they were affected.
