What You'll Do

As a Homecare Nurse (RN/LPN), you'll provide skilled, compassionate nursing care in the comfort of your patient's home. Working closely with physicians, case managers, and clinical leadership, you'll help patients achieve the highest quality of life while delivering personalized care tailored to their unique needs.

Key Responsibilities

  • Deliver skilled nursing care and administer treatments and medications according to physician orders and individualized care plans.
  • Monitor patient conditions and identify changes, concerns, or opportunities for intervention.
  • Collaborate with patients, families, physicians, and the interdisciplinary care team to ensure exceptional outcomes.
  • Utilize critical thinking and clinical judgment to support safe, effective patient care.
  • Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
  • Maintain professionalism, reliability, and punctuality in all assigned shifts.
  • Adhere to company policies, procedures, and best practices for patient care.

What We're Looking For

Qualifications

  • Active, unrestricted RN or LPN license in the state of practice.
  • Current CPR certification.
  • Current TB test results or chest X-ray documentation.
  • Reliable transportation.
  • Strong communication, organization, and time-management skills.
  • Ability to work independently while maintaining a collaborative approach to patient care.
  • Commitment to providing compassionate, patient-centered nursing care.

Physical Requirements

  • Ability to sit, stand, walk, bend, kneel, crouch, balance, reach, push, and pull as required to safely perform patient care duties.
  • Ability to lift and/or move up to 15 pounds frequently and up to 100 pounds occasionally when necessary.
  • Visual and auditory abilities sufficient to perform nursing responsibilities, with or without reasonable accommodation.
  • Ability to safely provide the level of physical assistance required for patient care and communicate any limitations to a supervisor promptly.
