Job Summary
The SLP or ST provides speech and language services to patients or clients as prescribed by the physician and in compliance with applicable laws, regulations and policies of MGA.
Duties and Responsibilities
- Seeks and obtains sufficient information necessary to function effectively.
- Provides individual patient/client services and determines priority needs for speech language by:
- Assessing and evaluating therapeutic, rehabilitative, and functional status.
- Identifying functional speech defects, potential for rehabilitation, and evaluating the home environment.
- Evaluating the need for and recommending communication devices and/or aides.
- Participating in the development of the total plan of care.
- Directing the speech-language pathology treatment plan of care.
- Regularly re-evaluating patient/client progress and revising the plan of care as necessary.
- Initiates speech language pathology and instructs other personnel in certain phases during which they may work with a patient/client
- Maintains communication regarding patient/client status with supervisory personnel.
- Collaborates with the physician and team members to develop, implement, and support the patient’s plan of care.
- Completes and maintains accurate clinical notes regarding patient evaluation and plan of care.
- Observes the patient/client for reactions to treatment or changes in condition, discusses and reports changes as necessary.
- Assists caregivers/families, other healthcare personnel in providing continuity of care.
- MGA Policies, Procedures, and Handbook are adhered to.
- Notes are completed accurately and submitted as per respective policies and procedures.
- Clinical documentation must be turned in weekly per agency guidelines.
- Represents themselves positively and professionally in the company and community.
- Adheres to dress code with a clean and neat professional appearance.
- Reports on time and as scheduled to complete work within the designated time.
- Participation in the Agency’s other mandated activities as requested by the Agency Director, such as In-services.
Requirements
- Master’s degree in Speech Language Pathology
- Current License and in good standing with the State Licensing Board
- Has Certificate of Clinical Competence in Speech Language Pathology by ASHA
- Successful completion of the appropriate screening test with a passing rate
- Current CPR certification per state regulations
- Professional experience required per state regulations
- Displays good observation skills and judgement as well as good oral and written communication skills
- Reliable Transportation
Physical Requirements
- Ability to regularly sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull, and reach with hands and arms, and perform other physical activities as deemed necessary by job duties.
- Vision and Hearing capabilities within normal limits, with or without aid.
- Ability to frequently lift and or move up to 15 pounds and occasionally lift or move 100 pounds or more if applicable.
- Ability to perform the anticipated level of physical assistance required to provide safe client care. If the anticipated level of assistance exceeds what the employee can safely provide, the supervisor is to be notified immediately.
Work Environment
- The work environment is highly variable, as care is generally provided in private homes.
- Lighting, acoustics, air quality, temperature, and environmental hazards are not predictable.
- The employee is occasionally exposed to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals.
- The employee is required to travel frequently to complete home health visits.
