Job Description

Responsibilities:
Clinical Training and Education: Provide instruction and training on clinical skills and procedures during the initial hiring process and continuously as needed, with an emphasis on the professional development of field staff.
Supervision of Home Care Personnel: Supervise all home care personnel, including non-licensed staff, LPNs, and RNs, ensuring appropriate task delegation, compliance with state guidelines, and adherence to best practices.
Coordination of Patient Care: Facilitate effective communication to ensure the seamless implementation of each patient’s Plan of Care.
Support and Guidance: Offer support, guidance, and education to patients, their families, and caregivers to promote optimal physical and emotional well-being.
Mandatory Reporting: Identify and report any observed or suspected patient abuse, neglect, or exploitation to the appropriate authorities as required by law for health professionals.
Qualifications
- Current state license as a Registered Nurse, BSN preferred.
- Minimum of 2 years of nursing experience in a Home Health or Acute setting required.
- Minimum 3-5 years of clinical management/supervision preferred.
- Demonstrate leadership capabilities with strong supervisory and interpersonal skills.
- Detail oriented, flexible, good organization, and time management skills.
